Root for Parents and Children

Registration is now open for Root for Parents and Children. This program is a first year program for families with children in the 1st - 3rd Grade

Dear Faith Formation Families,
We are excited for the new year of journeying together in our faith. To best facilitate the registration process, we are asking that you gather the necessary documents ahead of completing and returning your registration packets.

The following documents are required for all students.
____ Birth Certificate (original and copy)
____ Baptismal Certificate (original and copy)

The Registration packets will be available for pick up beginning on Monday, July 15, from 4 pm to 7pm at the Parish office.
____ Registration Form (Hard copy completed even if registered online)
____ Liability Form
____ Circle of Grace Signed Lesson (or opt out sheet)
____ Volunteer Form
____ Check or exact cash for registration fees (Registration fees can also be paid online via credit card, beginning on Monday, July 15, 2024)
